What is a Dobutamine Stress Echo (DSE)?

A Dobutamine Stress Echocardiography (DSE) is a specialized test that checks how your heart works when it's under stress — without physical exercise.

Instead of walking on a treadmill like in a TMT, you are given a safe medication called Dobutamine, which temporarily makes your heart beat faster and harder — similar to how it would during exercise. During this, a 2D Echo (ultrasound) is done to see how well your heart pumps under stress.

It helps the cardiologist understand:

  • If there are blocked or narrowed arteries (coronary artery disease)

  • Whether parts of the heart muscle are not receiving enough oxygen

  • Your heart’s pumping strength (ejection fraction) under stress

  • If you’re fit for surgery or recovery after a cardiac event

How is a DSE Done?

  1. ECG electrodes are placed on your chest

  2. An IV line is inserted into your arm

  3. A small dose of Dobutamine is slowly injected to increase heart rate

  4. 2D Echo images are taken at rest, during stress, and after recovery

  5. Your ECG, blood pressure, and symptoms are monitored throughout

  6. The test is stopped if target heart rate is reached or symptoms appear

  7. ⏱️ The entire test takes around 30 to 45 minutes, including preparation. You can go home after a short observation period.

Your cardiologist may advise a DSE if:

  • You cannot perform a treadmill test due to joint pain, age, or disability

  • You have chest pain or shortness of breath without clear ECG findings

  • There’s suspicion of coronary artery disease (CAD)

  • You are undergoing pre-surgical evaluation
