
Treadmill Testing (TMT)
What is a TMT (Treadmill Test)?
A Treadmill Test (TMT), also called an Exercise Stress Test, checks how your heart performs when it works harder during exercise. It records your ECG (electrocardiogram) while you walk on a treadmill whose speed and slope gradually increase.
It helps the cardiologist understand:
- Whether your heart gets enough blood and oxygen during exertion
- How your heart rate, rhythm, and blood pressure respond to exercise
- If there are signs of reduced blood flow (ischemia)
- Your fitness level and exercise tolerance
Think of a TMT as a heart fitness check under stress, showing how your heart performs in real-life conditions.


How is a TMT Done?
1. ECG stickers (electrodes) are placed on your chest.
2. You walk on the treadmill at a comfortable speed.
3. Every 3 minutes, the treadmill becomes slightly faster and steeper.
4. Your ECG, blood pressure, and pulse are continuously recorded.
5. The test stops if you reach your target heart rate, or if you develop chest pain, breathlessness, or fatigue.
The whole process takes around 15–20 minutes, including preparation. You can return to normal activity immediately after the test.
No injections, no pain, no radiation.
A few simple steps can make your TMT more comfortable and accurate:
1. Clothing: Wear loose, comfortable clothes and sports shoes.
2. Food: Avoid a heavy meal for 2–3 hours before the test. A light snack is fine.
3. Caffeine: Avoid tea, coffee, or energy drinks for at least 2 hours before the test.
4. Medications:
- Continue your usual medicines unless your cardiologist advises
otherwise.
- Sometimes, you may be asked to withhold beta-blockers (like metoprolol)
before the test — follow your doctor’s instructions.
5. Hydration: Drink a small amount of water before the test; don’t overhydrate.
6. Rest: Get adequate sleep the night before.
7. Avoid applying body lotion or oil on the chest area — it prevents ECG electrodes from sticking properly.
Your doctor or technician will explain everything again before starting the test, so you’ll feel relaxed and confident.

A TMT is recommended when you have:
- Chest pain or pressure during exertion
- Shortness of breath while walking or climbing stairs
- Suspected or known coronary artery disease
- Follow-up after angioplasty or bypass surgery
- Unexplained tiredness or reduced exercise tolerance
- Pre-fitness or pre-employment screening, especially after age 40
Even if ECG and Echo are normal, TMT can reveal exercise-induced heart issues that appear only under stress.
